The person submitting this registration form must be 18 years of age or older.
By submitting the form, you verify that the below information is accurate and you have read and understood NCMC's policies and procedures.
Submitting this form incurs a $15 registration fee. You will not be charged until you complete your intake session with the Music Therapist and your prescribed course(s) of treatment has been processed by our registrar. If you only enroll in the ND Jam Band, you will be except from the registration fee, and it will be removed from your balance. If you go on to enroll in Adaptive Music Lessons or Individual Music Therapy, you will receive email invoices from NCMC to pay your balance based on amount owed, financial aid status, and payment plan chosen.
